probably be dilated, so bring sunglasses if you have them. If not, we will provide a disposable pair.

Bring the following to your first appointment:
- Completed New Patient Form
- Completed New Patient Questionnaire
- Completed Financial Agreement
- List of medications
- Eye drops you are using
- Glasses or contact lenses
- Insurance card(s)
- Primary care physician referral
- Sunglasses
Appointments
We see patients on weekdays and Saturday mornings. Our staff will do everything possible to accommodate you with a convenient time.
Emergencies
In an emergency, call our office day or night for instructions. You may be told to go to Lutheran General Hospital in Park Ridge. Upon arrival, please inform the hospital staff that you are a patient of our practice.
Payment
We accept Medicare assignment and participate in most HMOs and PPOs. If your insurance plan requires a referral, please obtain it so you can bring it to your initial exam. Cosmetic and refractive procedures are not covered under insurance plans, requiring payment at the time of service. We accept personal checks, Visa, MasterCard and Discover. If you are considering a cosmetic procedure, LASIK or INTACS, ask about our interest-free financing options.
